Ordering Information
  1. Indicate quantity of tags (in increments of 100).
  2. Indicate style number (as shown in price box).
  3. Indicate copy and layout for tag.
  4. Specify color and copy by color name (serialize numbers in black only).
  5. Specify beginning number of sequence if serializing desired, i.e., numbers to start at 000501 and end at 0014699. Add 10%.
